In Arizona, obtaining a disability parking placard is a straightforward process:

1. Complete the application.
   Use the Arizona Individual Disability Plate/Placard Application
2. Have a qualified healthcare provider certify your disability.
   The medical certification may be completed by a:
   • Physician (MD or DO)
   • Physician assistant (PA)
   • Nurse practitioner (NP)
   • Physical therapist
   • Hospital administrator (in certain circumstances) 
3. Submit the application.
   You can:
   • Mail it to the Arizona Motor Vehicle Division (MVD)
   • Fax or email it if you are applying for a placard only
   • Take it to an MVD office or an authorized third-party MVD provider 
4. Receive your placard.
   • Permanent placards: Free and, under current Arizona law, do not expire as long as you continue to qualify.
   • Temporary placards: Free and valid for 6 months. A new medical certification is required if you still need one after it expires. 

A few additional points:

• The placard belongs to the person with the disability, not the vehicle. It may be used in any vehicle in which that person is the driver or passenger.
• If needed, Arizona may issue up to three placards for one eligible individual with MVD approval.
